In his new position, Milde will be responsible for planning, communications, and strategic management of the ministry. He will report directly to the minister. The Federal Ministry of Health is not the first ministry he has worked for. Milde, a member of the CDU since 1993, previously served as a department head at the Federal Ministry of Transport, where he was responsible for energy, climate, and environmental protection issues.

The political scientist also served as federal executive director of the Junge Union (Young Union) from 2003 and as a personal assistant to former Chancellor Helmut Kohl (CDU) from 2006. From 2014 to 2019, Milde was editor of the specialist magazine Politik & Kommunikation (Politics & Communication). He is also an author, focusing on topics such as the coronavirus pandemic, the future of the EU, and global economic changes, such as pharmaceutical supply chain problems. For a book project in 2021, Milde traveled several thousand kilometers across Germany with then-CDU General Secretary Paul Ziemiak.

Milde takes over from Boris Velter, who served as head of the executive staff at the Federal Ministry of Health under Federal Health Minister Karl Lauterbach (SPD) from 2021 to 2025. In his role as federal chairman of the Social Democrats' Association in the Health Care Sector , Velter expressed skepticism about the allocation of portfolios in a letter to members in April . He called it a "major mistake" that the Federal Ministry of Health would go to the CDU in the new government.
